New Patient Appointment Request

Thanks for your interest in Tennessee Reproductive Medicine (TRM). To request your initial appointment, please fill out the form below and complete the following steps:

• Step 1: Please fill out our appointment request form below. Please allow 2-3 business days for your chart to be created.
• Step 2: Once your chart has been created, you’ll receive an invite via email to create a patient portal account. Please check your inbox, junk and spam folders for the invite. Once received, you will have 48 hours to set up your portal account.
• Step 3: Log into your portal to sign our patient consents. After signing, please call the practice at 423-509-8094 to schedule your first appointment.
• Male patients: We must have an order from your referring provider before we’re able to schedule your appointment.
