How to refer your patients to TRM

To refer your patient to TRM please fax all pertinent notes and lab work to (423) 643-0699.

Alternatively pre-printed referral forms may be obtained by contacting our office at 423-509-8094 and requesting that a packet of the forms be either mailed or delivered to your office.
