Tennessee Reproductive Medicine’s Hope for the Holidays Giveaway

Rules and Guidelines

Tennessee Reproductive Medicine (TRM) is holding the Hope for the Holidays Giveaway to provide an individual or couple struggling with fertility the opportunity to complete a free in vitro fertilization (IVF) cycle in 2018. All entrants in the contest agree to comply unconditionally with all the provisions set out in these rules and guidelines – any entrant who does not comply will be considered ineligible for participation. TRM shall have the right, but not the obligation, to monitor the contest and determine compliance with these rules and guidelines, and the right at its sole discretion to refuse and remove any applicant that it finds to be in violation of these rules and guidelines or otherwise unlawful and objectionable.

Eligibility

The Hope for the Holidays Giveaway is open to all eligible applicants ages 18 years or older, including past and current TRM patients. Applicants must be self-pay with no insurance coverage for infertility treatment. The winner will undergo medical evaluation to ensure that IVF is the recommended and appropriate course of treatment for the patient. Employees of TRM and their family members and those living in their immediate households, whether or not related, are ineligible. There is no entry fee.

What’s Included

The winner of TRM’s Hope for the Holidays Giveaway receives one free in vitro fertilization (IVF) cycle to be completed on or before December 31, 2018. The prize is the standard IVF cycle offered by TRM, including treatment scans, laboratory testing, egg and sperm retrieval, fertilization and embryo transfer. Costs for preimplantation genetic screening (PGS), donor or surrogacy services, and/or procedures resulting from unforeseen complications are not included in the giveaway. Auxiliary costs including but not limited to travel to and from the clinic, food, lodging and vehicle rental are also not included in the giveaway.

How to Participate

The contest is offered electronically via the Internet. Entrants may enter the contest by following the link in to TRM’s website, TRMBaby.com/giveaway, and completing the entry form questionnaire online.

Permissions & terms
: Entrants must complete the required questionnaire and assert that their application entries are accurate.

In addition, winning entrants grant TRM royalty-free, perpetual rights to announce winner information or share their story to other websites sites, to social media, to its website, and to any other company that promotes Tennessee Reproductive Medicine, including, without limitation, to any news media, all of whom may not be subject to the same federal health privacy laws.

Once published or released, Tennessee Reproductive Medicine has no control over who views or uses the information.

Winner Selection & Notification

The winner will be awarded via random drawing from a pool of pre-screened applicants. The pool of pre-screened applicants will be determined based on the information provided on the contest application and the ability of the applicant to safely undergo such treatment. Should the information in the application be determined to be falsified or incorrect at any time during the contest, up to and including beginning treatment if chosen as the winner, TRM reserves the right to disqualify the application from the contest.

Judging: Entries will be pre-screened and reviewed by a panel of judges chosen by TRM and will include TRM employees and outside community representatives. The panel will nominate the top candidates (no less than 10% of all entries) for a random drawing.

Random drawing & winner selection: One (1) winner will be notified on or around December 15, 2017. The decisions of the judging panel and the random selection of the winner are final and binding, and not subject to appeal.

Prize acceptance: Winner of the in vitro fertilization (IVF) cycle prize must undergo the IVF cycle on or before December 31, 2018 at TRM’s office at 6031 Shallowford Road, Suite 101 in Chattanooga, TN. The prize is nontransferable. In the event any winner waives their right to receive the prize or the winner is medically incapable of undergoing treatment (i.e. inability to undergo anesthesia, ovaries are not accessible for aspiration of oocytes, etc. based on results of diagnostic testing) at the discretion of TRM physicians and ancillary providers, TRM reserves the right to award any waived prize to another eligible entry.

Disclaimer of Liability

All entrant(s) by registering with the Hope for the Holidays Giveaway agree(s) that TRM, its agents, representatives, employees, and contractors, are released and discharged from and against all and any costs, claims, damages, liabilities, and expenses of any sort arising out of, or generally relating to: submission of the material for the contest and participation in it, the use of the material by TRM, the decisions made or actions taken by TRM in connection to the conduct and administration of the contest and the acceptance and use on their part of the prize.

The entrant(s) waive(s) any right to seek injunctive action or other equitable relief relating to any claim against TRM and its agents, representatives, employees, and contractors.

Changes to the Rules and Guidelines

TRM shall have the right at any time and without prior notice to modify, suspend, or terminate the contest or the Rules and Guidelines, or any part thereof; and/or impose new rules or guidelines as it considers appropriate. Such changes, modifications, additions, or deletions shall be effective immediately upon notice thereof, which may be given by means including, but not limited to, posting on the TRM website, sending electronic or conventional mail to the entrants, or by any other means by which the entrants obtain notice thereof.
